Have you ever noticed that your tennis elbow doesn’t just stay in your elbow anymore?
Maybe it started as pain on the outside of the elbow…But now your forearm feels tight.Your wrist aches.Your shoulder feels sore.Your neck feels stiff.
And suddenly you start wondering:
“Is my injury getting worse?”
This is actually something I hear all the time from people struggling with persistent tennis elbow — especially when the pain has been around for longer than 3 months.
And the answer may surprise you.
It’s Not Always About More Tissue Damage
Many people assume that if pain spreads, the tendon must be tearing more or deteriorating further.
But often, that’s not what’s happening at all.
When pain persists for a long period of time, changes can occur within the nervous system and brain. The body can become more protective and more sensitive to movement, load, stress, and even anticipation of pain.
This is one reason why some people:
Rest for months but don’t improve
Try endless stretches and massage with minimal success
Feel pain moving beyond the original injury site
Start becoming fearful of movement
This is also why I talk so much about brain pain science and central sensitization in my work.
Because healing chronic tendon pain is rarely just about the tendon itself.
Tennis Elbow Healing Requires a Bigger Picture Approach

Tendon loading strategies
Nervous system regulation
Movement patterns
